The following matters, in addition to matters listed in Article 666, shall be entered into a personal insurance policy: <Amended by Act No. 4470, Dec. 31, 1991>
1. The kind of an insurance contract;
2. The domicile, full name, and birth date of the insured;
3. The address, full name, and birth date of the beneficiary of insurance, if such beneficiary has been determined.
